D4U Medical – www.d4u.org.uk (These Terms reflect operational practices in place since the launch of www.d4u.org.uk)These Terms and Conditions govern all services provided by D4U Medical. By booking an appointment or using our website, you agree to the following terms in full. If you do not agree, you should not proceed with using our services.
1. Definitions
- “We”, “Us”, “Our” refers to D4U Medical
- “You”, “Patient”, “Client” refers to the individual using our services or attending an appointment
- “Appointment” refers to any booking made for a medical consultation, assessment, or examination
2. Booking and Confirmation
- Bookings can be made through our website or via telephone
- Once booked, you will receive a confirmation email
- If you do not receive this email, it is your responsibility to contact our office by phone to confirm
- Bookings are considered confirmed only upon receipt of this confirmation email
3. Documentation Requirements
Patients must bring the following to their appointment:
- One photographic ID (passport or UK driving licence)
- One proof of address dated within the last 6 months (utility bill, bank statement, or council tax bill)
Failure to bring both documents may result in cancellation of the appointment and loss of the booking fee.
4. Medical Records
- Some medicals require full medical records from birth
- These must be requested from your registered GP and can take up to 28 days to be provided
- It is your responsibility to ensure you bring the required records
- If the required records are not available at the time of your appointment, the assessment may be cancelled and fees forfeited
4A. Local Authority Medical Record Requirements
If you are attending a medical for taxi, private hire, or hackney carriage licensing, you must check with your local council what documentation is required. This may include:
- Full GP medical records
- A summary of medical records
- An NHS health record
These requirements vary by council. We strongly advise confirming this with your licensing authority before your appointment. If your council’s policy states you require full medical records, we request that you contact your GP to obtain your full medical records from birth.Failure to bring the correct records may result in cancellation and loss of your booking fee.
5. Form Completion
- Patients must complete personal information on the relevant forms prior to the appointment
- Forms may be sent electronically and must be returned in advance or printed and brought on the day
- Failure to complete the required forms may cause delays or cancellation
6. Attendance and Timekeeping
- Arrive at least 10 minutes before your appointment
- If you are more than 10 minutes late, your appointment may be cancelled or rescheduled without refund
- Missed appointments or no-shows are non-refundable
7. Cancellations and Rescheduling
- A minimum of 48 hours’ notice is required for cancellations or changes
- Cancellations within 48 hours of the appointment will result in the loss of the full fee
- Rescheduling is subject to availability
- Repeated changes may incur an admin fee
8. Refund Policy
Refunds are granted at our discretion. No refunds will be given in the following cases:
- Failure to bring required ID or medical records
- Late arrival or missed appointment
- Incomplete or missing forms
- Where the examining doctor determines that certification cannot be issued
Booking fees are non-refundable once confirmed, unless D4U cancels the appointment.
9. Medical Decision and Certification
- All assessments are conducted by GMC-registered doctors
- Certification is based on medical standards set by governing bodies
- Certification is not guaranteed
- If you do not meet the required medical criteria, no certificate will be issued and no refund will be provided
10. Liability Disclaimer
We accept no liability for:
- Loss or delay due to refusal of certification
- Delays caused by incomplete or incorrect documentation
- Consequences resulting from withheld medical information
- Incorrect, false, or misleading information provided by the patient
Client responsibility: You are solely responsible for the accuracy and completeness of the information you provide. If a certificate is issued based on withheld or false information, it may be withdrawn or deemed invalid.
11. Website Use
- Website content is for general information only
- We do not guarantee uninterrupted access or error-free functionality
- All content remains the property of D4U Medical
12. Data Protection
- We comply with the UK GDPR and Data Protection Act 2018
- Data is securely stored and only used in relation to your booking or medical assessment
- We do not share your personal information without consent unless required by law
13. Governing Law
These Terms are governed by the laws of England and Wales. Any disputes will be resolved under the exclusive jurisdiction of the English courts.
14. Terms Authoring Disclaimer
These Terms and Conditions have been prepared internally by D4U Medical for operational use. They do not constitute legal advice. If you have questions about your rights or responsibilities, you should seek independent legal advice.
15. Complaints
If you are dissatisfied with any aspect of our service, please raise your concern with our team by calling 0330 229 3304. We aim to resolve all complaints fairly and promptly. Where necessary, we may provide a written response within 10 working days.
16. Right to Refuse Service
We reserve the right to refuse or terminate an appointment at any time if a patient is abusive, threatening, under the influence of drugs or alcohol, or otherwise unfit for examination. No refunds will be issued in such cases.
17. Chaperone Policy
Patients may request a chaperone to be present during their medical assessment. Please inform us at the time of booking if this is required. We may assign a chaperone at our discretion for specific appointments.
18. Rebooking
If your appointment cannot proceed due to missing documents, incomplete forms, or insufficient medical records, you may be required to rebook and pay the full fee again.
