How do I book a medical?

What does a driver medical assessment involve?
Our doctors will assess fitness to drive based on DVLA guidelines.
Generally, this will involve the following:
- Vision assessment
- Blood pressure monitoring
- Details regarding your medical history
- List of current medications
Once these sections are completed, the driver will complete the declaration to confirm all the information is correct. Any forms that are incomplete or contain errors will be returned by the DVLA.

What do I need to bring to my appointment?
- D4 Medical form or any relevant form/document based on the medical required
- 1 form of photographic ID either driving licence or passport
- Your glasses/lenses and eyesight prescription (within 2yrs, if diabetic within 1yr)
- List of current medications including doses and frequency
- Any hospital letters from specialists who you may be under the care of
Will I know if I have passed or failed my medical on the day?

Our D4U doctors are not responsible in determining whether you pass or fail the medical. They do however work alongside the DVLA to aid the decision-making process.
Upon completion of medical, the D4 form should be posted to the DVLA who will ultimately make the decision.
Rest assured, you will be informed on the day if there are any causes for concern when the examination is carried out.
What if I suffer from a medical condition?

There are stricter guidelines with regards to driving group 2 vehicles such as lorries and buses as compared with Group 1 vehicles such as cars and motorbikes. Due to the size, weight and carriage/load capacity of these vehicles they pose a much greater risk towards your own as well as public safety.

Once I have sent off the forms, how long will it take to get my updated licence back from the DVLA?

On average it takes between 2-3 weeks for an application to be processed and completed.
Can I cancel my appointment?

We can reschedule an appointment dependant on availability at no extra cost. Please be aware once a booking is made though our online portal, we cannot provide a refund.
Can I make a payment on the day?

Although we encourage online booking, payments can be made on the day at a discounted rate. Please enquire within.
Can you provide medicals at my business site?
